LK Farm – Boarding & Dog Daycare Terms & Conditions
This agreement is valid from the date signed, for the dates stated, unless updated information is provided by the owner or required by LK Farm. By signing this agreement, I authorize LK Farm ("the Facility") to provide cage-free pet boarding and/or dog daycare services for my pet(s) and agree to the following:
1. Health & Vaccinations
I certify that my pet(s) are in good health and are up to date on all required vaccinations, including rabies, distemper/parvo, bordetella (kennel cough), and flea/tick prevention, as outlined by LK Farm. I will provide updated vaccination records annually or as requested. I understand that bordetella (kennel cough) vaccine is not 100% effective against all strains of virus' but will reduce the severity of illness in most cases.
2. Behavior & Right of Refusal
I affirm that my pet(s) have not exhibited any aggressive or dangerous behavior towards other animals or people. I understand that LK Farm reserves the right to refuse or terminate services at any time if a pet displays signs of illness, aggression, or behavioral issues that may pose a risk to other pets, staff, or property.
3. Veterinary Care Authorization
In the event of illness or injury, LK Farm will make reasonable efforts to contact me or my designated emergency contact. If contact cannot be made promptly, I authorize LK Farm to seek medical treatment for my pet(s) at a local veterinarian of their choice. I agree to assume full financial responsibility for any and all expenses incurred, including but not limited to emergency care, diagnostics, medications, or hospitalization.
4. Assumption of Risk & Liability Release
I acknowledge that I am enrolling my pet(s) in daycare and/or boarding at LK Farm voluntarily and at my own risk. I agree that LK Farm and its owners, staff, and agents shall not be held liable for any injury, illness, loss, or damage to my pet(s), property, or person, except in cases of gross negligence or willful misconduct.
5. Owner Indemnification
I agree to indemnify and hold LK Farm harmless for any injury, illness, loss, damage, or expenses caused by my pet(s) while in the care of LK Farm, including but not limited to harm to other pets, people, or property.
6. Socialization & Behavioral Considerations
LK Farm provides a supervised, cage-free environment where pets may interact and socialize with one another.
I acknowledge that there are risks and benefits associated with pets socializing in a group or cage-free setting, and I agree that the benefits outweigh the risks.
I voluntarily accept these risks, which may include altercations, minor injuries (such as scratches or nicks), stress-related behaviors, or the potential transmission of illnesses despite all reasonable precautions.
I understand that dogs naturally play and may sustain injuries. While LK Farm monitors all group play and social interactions closely, I acknowledge that accidents and injuries may occur despite supervision.
LK Farm staff may, at their sole discretion, separate my pet(s) from others or adjust activities if necessary for safety or behavioral reasons.
7. Feeding & Special Care
I will provide clear instructions and all necessary supplies for my pet(s)’ diet, feeding schedule, and any special care (e.g., medications, allergies, health conditions). LK Farm will make all reasonable efforts to follow these instructions but shall not be held liable for allergic reactions, digestive issues, or other adverse effects.
8. Drop-Off, Pick-Up & Scheduling
I understand that LK Farm operates on a schedule for daycare and boarding arrivals and departures. Late pickups may incur a fee, and any pet not picked up as scheduled may be transferred to boarding at my expense.
9. Cancellations
I understand that boarding or daycare reservations must be cancelled at least 24 hours prior to the start of the reservation.
Cancellations made less than 24 hours before the scheduled start time may be subject to a cancellation fee equal to one day of the reserved service.
No-shows may result in full charges for the reserved stay or daycare session.
10. Media & Promotion
I grant LK Farm permission to photograph or video my pet(s) during their time in daycare or boarding, and I consent to the use of these images for marketing, social media, or promotional purposes. If I do not wish for my pet(s)’ image(s) to be used, I will notify LK Farm in writing prior to attendance.
